- I understand a tattoo is permanent, and removal is costly and may not be complete.
- I have checked the design, spelling, size, and placement, and I approve them.
- I understand there are risks, including infection, allergic reaction, and scarring, and that results vary with skin.
- I have answered the health questions truthfully and will tell the artist if anything changes.
- I have received aftercare instructions and understand healing is my responsibility.
- I give the studio permission to photograph the tattoo and use the photos in its portfolio and on social media.
- I release the studio and its artists from claims arising from the procedure, except where the law does not allow it.

Questions artists ask
What should a tattoo consent form include?
Client details and ID check, health questions, an acknowledgement that the tattoo is permanent and carries risks, approval of the design and placement, and signatures. Many artists add a photo release and an aftercare acknowledgement.
Can a client sign the consent form online?
Yes. Many artists send it before the appointment so the client reads it at home and the session starts on time. You still check photo ID in person.
Is this form legally enough on its own?
It is a starting point, not legal advice. Health and age rules differ by province, state, and country, and some places require specific wording. Check with your local health authority.
