Tattoo consent forms online, signed before they walk in
The clipboard at the door costs you ten minutes every session and gives you a stack of paper to store. Moving the form onto the client's phone before the appointment fixes both.
Why do it before the appointment
When someone fills the form out in the chair, they're nervous, they rush it, and you find out about the blood thinners after the stencil's already on. Sending it the day it books gives them time to answer properly, and gives you time to read it.
Read it before the day
A signed form you only look at in the chair doesn't help much. Skim it the day before. If something needs a conversation, like a medication or a skin condition near the placement, message them ahead of time instead of finding out with the machine in your hand.
What a consent form usually covers
Requirements depend on your city, province, or state, so check with your local health authority for what yours has to include. Most artists ask about:
- Name, date of birth, and confirmation they're of age
- Health questions that affect the session: medications, blood thinners, pregnancy, skin conditions, allergies
- Whether they've had anything to drink
- That they've checked the design, spelling, and placement
- That healing is partly on them, and they got the aftercare
- Whether you can post photos of the piece
Setting it up on Studio Haven
Consent forms live under Forms. Write or copy in your waiver in the Forms Library, then go to Client journey and set it as the consent form under Booking confirmed. Once a booking's confirmed, the client can sign it from their portal on their phone, and it stays attached to that booking.
You'll still check ID in person. The form means the first ten minutes of the session go to the tattoo instead of paperwork.

Are digital tattoo waivers allowed?
In many places, yes, but it depends on where you work. Your local health authority or inspector is the one to ask. This isn't legal advice, and we'd rather you check than guess.
Can I use the paper form I already have?
Yes. Copy the same questions and wording into the online form. You don't need to rewrite anything your inspector has already signed off on.
Do I still need to check ID?
Yes. The form collects the answers. Checking the ID against the person in your chair is still your job.
